What are my first steps?

You may email: [email protected], use the contact form provided, or call in order to schedule an appointment or a consultation. From there, you will recieve access to our online portal with intake paperwork, documents, important resources, appointment reminders and scheduling options for established clients.

What forms of payment do you accept?

We accept all major credit cards, cash, check, and HSA or FSA (please check with your insurance to see if you have a HSA or FSA). We also accept the following insurances:

***NEW***

We are now able to submit your Out of Network therapy claims for you! This means, that if you have an insurance company that we are not credentialed with, you will pay us directly up front, and we will submit paperwork on your behalf to your insurance company for them to reimburse you some, or all, of your out of pocket costs. Typical reimbursement is 80% of your rate.

What you need to know:

  • Your insurance information must be on file with us.

  • You will be paid from your insurance company directly if you are out of network.

  • Your deductible still might apply

  • We accept no responsibility for how much your particular insurance company will reimburse you- we are not privy to this information ahead of time since we aren't in network with them.

Insurance companies we are IN Network with:

  • United Healthcare

  • UMR

  • Optum

  • Humana

  • Bind/Surest

  • BlueCross and BlueShield

  • Anthem

  • Out of Network

  • Private Pay

What is the Good Faith Estimate?

A new law was passed that requires all healthcare providers to provide a Good Faith Estimate.  Here is what that means for you:

  • You will receive a Good Faith Estimate for the total expected cost of your services if you choose to private pay for services. This is just an estimate and will be updated if there are any changes.

  • You can ask for a Good Faith Estimate at anytime. We will provide you with one before your first session. You can also request one before scheduling.

  • The Good Faith Estimate gives you the right to dispute a bill that is at least $400 more than the Good Faith Estimate.

  • You can save or take a picture of your Good Faith estimate for your reference. For any questions about your right to a Good Faith Estimate, you can visit www.cms.gov/nosurprises or call our office at 502-276-5736

Payment is expected at time of service. Please contact your insurance company prior to your appointment if you are uncertain whether mental health services are covered.
